Please fill out the form below. Include the last name of the family and the parents names in the first two fields so we know who is responsible for which children (in case last names differ). As you add your names, the totals will automatically appear in the final two fields. 

 

When you are finished, please click the print button. You can either print this page out and mail it to Annette Lomax or you can save it as a pdf and email it to Annette Lomax. Along with the form you may either mail a 40% deposit check or send 40% via Venmo or GooglePay to netsylomax@gmail. (see contact link). Do not make the check out to "Family Camp", but rather you can write "Family Camp" at the bottom of your check. As we receive your forms and deposits, we will update a list of who is attending on the 2023 Info page so check back often.
